I. ROLL CALL/CALL TO ORDER
Chairman Raymond Buschmann called the meeting to order at 6:30 P.M.
Aldermen Melanie Rummel and Michelle Moreno were in attendance.
Staff in attendance included Michael Thomas, Director of Public Works; Dan
Martin, Superintendent of Public Works; Bob Ells, Superintendent of Engineering
and Chuck Myers, Superintendent of Parks & Forestry.
Also in attendance was First Ward Alderman, Prue Beidler.
II. APPROVAL OF THE MARCH 12, 2019 PUBLIC WORKS COMMITTEE MEETING MINUTES
Alderman Rummel moved to approve the March 12, 2019 Public Works
Committee meeting minutes subject to the typo identified by Chairman
Buschmann. Alderman Moreno seconded the motion, which carried
unanimously.
III. DISCUSSION ON THE FY ’20 DEERPATH STREETSCAPE DESIGN PROCESS – CHUCK
MYERS
Superintendent Myers explained that in FY19, City Landscape Architectural
Consultant, Craig Bergmann Landscape Design, completed a conceptual plan
for Deerpath Road Streetscape. The plan was completed with Bergmann
working closely with City staff and the Civic Beautification Committee. As part of
FY20 CIP budgeted funds, $150,000 was allocated for a full design of the
Deerpath Streetscape Project.
Superintendent Myers and Alderman Beidler provided the Committee with an
overview of the project and the past discussions about the need for the project.
Superintendent Myers explained that once design is completed the
City can proceed in funding the project through its own means and/or the City
can apply for a federal grant. Superintendent Myers reviewed the below chart:

Superintendent Myers reviewed the Illinois Transportation Enhancement Program
(ITEP). He explained that ITEP provides funding for community-based projects
that expand travel options and enhance the transportation experience by
improving the cultural, historic, aesthetic, and environmental aspects of our
transportation infrastructure. He explained that ITEP is on a two-year cycle with
the next call for proposals due December 6, 2019. Awards will be announced in
May, 2020. Superintendent Myers explained that if the City proceeds in
submitting and obtaining an ITEP grant, significant time will be added to the
project due to the grant’s IDOT requirements in design development and
approval. He explained that if the grant is awarded, significant cost savings
estimated at $736,000 would be realized. Superintendent Myers reviewed the
below conceptual project schedule if the City was to proceed in submitting and
obtaining an ITEP grant.
Superintendent Myers explained that City staff is seeking the Committees
direction in how the City should proceed in funding the project. The Committee,
City staff, and Alderman Beidler discussed the project, project funding, project
timing, project engineering, and submitting an ITEP grant project proposal.
There was consensus amongst the Committee that staff should proceed in
conducting an RFP process to identify a landscaping and engineering firm to
complete phase I engineering in preparation to submit a project proposal
through ITEP.
IV. REVIEW & RECOMMEND APPROVAL TO CITY COUNCIL THE CITY ENGINEER
SERVICES AGREEMENT – BOB ELLS
Superintendent Ells explained that the City’s current agreement with Gewalt
Hamilton to provide City engineering services will be ending on April 30, 2019. As
part of the process to form a new agreement, City staff conducted and
completed a Request for Qualifications (RFQ) process. As part of the RFQ
process, two engineering firms, Bleck Engineering and Gewalt Hamilton,
submitted proposals reviewing their firms. Both of the firms were also interviewed
by a selection committee comprised of City staff. Following the interviews, the

selection committee recommended selecting Gewalt Hamilton to fulfill the next
City Engineering services contract.
Superintendent Ells explained that Gewalt Hamilton has provided Engineering
services for the City over the past six years. Gewalt has been very successful in
providing those services.
Superintendent Ells reviewed Gewalt Hamilton’s hourly rates and key personnel
exhibit that was included in the Committee packet. He explained that the City
budgets an estimated annual amount of $43,000 for City engineering services.
The Committee and City staff had discussions on the services provided by the
City Engineer, past experiences with Gewalt Hamilton, Gewalt Hamilton’s
experience with other municipalities, and the proposed contract length and
terms.
Alderman Rummel motioned to recommend approval to City Council the City
Engineer Services Agreement. Alderman Moreno seconded the motion, which
carried unanimously.
V. REVIEW & RECOMMEND APPROVAL TO CITY COUNCIL THE AWARD OF THE 2019
SANITARY SEWER INFLOW/INFILTRATION TESTING PROJECT TO RJN GROUP IN THE
TOTAL NOT-TO-EXCEED AMOUNT OF $70,323 – BOB ELLS
Superintendent Ells reviewed the City’s annual smoke testing program. He
explained that the City has had portions of the sanitary sewer system become
overwhelmed with storm water and cause backups into residents’ properties. The
focus of this project will be to test portions of the City’s sanitary sewer system to
look for instances of inflow and/or infiltration. If testing shows inflow from private
property into the City’s sanitary sewer, the City will require residents to
disconnect the illegal connection. If infiltration is shown, the City will line such
sanitary sewers and make any necessary repairs. He explained that program bids
were received on April 12, 2018 and that a total of two firms submitted bids for
the project. RJN group submitted the low bid. RJN Group has agreed to utilize
the 2018 pricing for the 2019 program.
Superintendent Ells reviewed the below smoke testing funding chart:

Superintendent Ells explained that the following areas will be smoked tested in
the late summer / early fall: Within the Old Elm / Forest Hill storm sewer and water
main project area, Westminster from Green Bay Road to Oakwood Avenue, and
a small area near the Spruce & Sheridan sanitary sewer lift station.
The Committee and City staff discussed the smoke testing process that included
following up on the necessary public and private repairs.
The Committee and City staff discussed the potential grant opportunities to help
fund public repairs.
Alderman Moreno motioned to recommend approval to City Council the award
of the 2019 sanitary sewer inflow/infiltration testing project to RJN Group in the
total not-to-exceed amount of $70,323. Alderman Rummel seconded the
motion, which carried unanimously.
VI. REVIEW & RECOMMEND APPROVAL TO CITY COUNCIL THE FY ’20 SEWER LINING
AGREEMENT – BOB ELLS
Superintendent Ells reviewed the City’s annual sewer lining program. He
explained that the City participates in the Municipal Partnership Initiative’s (MPI)
joint bid to obtain sewer lining pricing from firms. As a result of the bid process in
2018 under MPI , with Mundlein and Highland Park, a contract was executed with
Hoerr Construction. The contract allows for three years of lining; this is the second
of the three years. He explained that the 2019 contract unit prices will increase
1.8% over last year. He explained that Hoerr Construction has worked in Lake
Forest on previous lining contracts with satisfactory results. He reviewed the
below sewer lining funding chart:
The Committee and staff had discussions about the lining process, its
effectiveness, and where lining has been completed in the past.
Alderman Moreno motioned to recommend approval to City Council the FY ’20
sewer lining agreement. Alderman Rummel seconded the motion, which carried
unanimously.
VII. REVIEW & RECOMMEND APPROVAL TO CITY COUNCIL THE DES PLAINES RIVER
WATERSHED-BASED PLAN – BOB ELLS
Superintendent Ells explained that the Des Plaines River Watershed Workgroup
(DRWW) is a voluntary, dues paying organization with a mission to bring together

a diverse coalition of stakeholders to work together to improve water quality in
the Des Plaines River and its tributaries in a cost effective manner to meet Illinois
EPA requirements. He explained that the City of Lake Forest does participate in
and is a member of the workgroup.
He explained that Lake County Stormwater Management Commission took the
lead to develop a watershed based plan for the DPR planning area. The
purpose of their effort was to develop a plan to reduce the impacts of water
pollution and flood damage, restore watershed lakes, streams, and wetlands to
a healthy condition, and provide opportunities for watershed stakeholders to
have a significant role in the process.
Superintendent Ells explained that the workgroup is seeking each participating
municipality’s public approval of the plan through a standard resolution shared
with the member municipalities.
The Committee and City staff had discussions about staff’s involvement in
developing the plan, its impact, and the area of Lake Forest that is located
within the watershed.
The Committee and City staff discussed the City’s required IEPA NPDES
documentation.
The Committee and City staff discussed deicing programs and stormwater
management.
Alderman Rummel motioned to recommend approval to City Council the Des
Plaines River Watershed-Based Plan. Alderman Moreno seconded the motion,
which carried unanimously.
VIII. REVIEW & RECOMMEND APPROVAL TO CITY COUNCIL THE LOW BIDDER FOR THE FY
‘20 OLD ELM & TIMBER STORM SEWER AND FOREST HILL WATERMAIN PROJECT – BOB
ELLS
Director Thomas reviewed FY 20 Old Elm & Timber storm sewer and Forest Hill
watermain project. He explained that In November,, 2014, Baxter & Woodman
performed a comprehensive storm water drainage study for The City of Lake
Forest. The report identified thirteen locations to address needed storm sewer
improvements. He explained that the highest priority project was constructed
during the summer of 2017. The next project, Old Elm & Timber, identified that the
existing storm sewer needs to be enlarged to meet the minimum requirements of
the Lake County Watershed Development Ordinance in order to handle a 10-
year storm event. The modeling for this study area showed the existing sewer
having a capacity for a 5-year event. By enlarging the storm sewer along Old
Elm, the depth of flooding on Forest Hill Road west of Timber Lane will be
reduced.

Additionally, Director Thomas explained, Forest Hill had multiple water main
breaks over the past five years. Staff planned to replace Forest Hill’s water main
and, thus the project has been listed in the annual capital project plan. It has
been staff’s intention to combine these two projects to minimize the disruption to
surrounding residents and to maximize efficiencies by having one contractor
install both pipes at the same time.
Director Thomas explained that if approved the project would commence on or
about June 10th and be completed by the end of August. If approved by City
Council, a letter will be sent to the residents within the project limits notifying
them of what the project entails, what they can expect, and who to contact if
they have any questions or concerns. The City’s website, under “Construction
Updates”, will also provide weekly details on the construction schedule.
Director Thomas reviewed the following bids that were received by the City:

Director Thomas explained that the storm sewer portion of the project came in
over budget at $351,889. According to Director Thomas, the project was over
budget due to the construction estimate being formed at a time without having
a final design. He explained City reserves has to be used to cover the budget
overage. Use of the reserves would need to be approved by City Council.
Director Thomas explained that the low bidder, Copenhaver Construction,
submitted references per the bid documents. References were checked with
unfavorable results, primarily focusing on the contractor beginning the job but
not returning to complete it until very late in the contract period and sometimes
past the contractual end date. References also noted that staff spent a
significant amount of time overseeing the construction to ensure items were
being installed per the specifications.

Director Thomas explained that staff is recommending the contract be awarded
to the lowest responsive and responsible bidder, Bolder Contractors. Bolder
Contractors references were found to be favorable by the City’s engineering
staff. Bolder Contractors has completed many projects this size and larger with
many of the surrounding municipalities. Each of the municipalities found Bolder
Contractors to be reliable, completed the projects on time, and without
constant oversight by engineering staff.
Director Thomas explained that like all other underground or large capital
projects, staff is requesting a 10% contingency be approved for any unforeseen
circumstance that may arise during construction. If the contingency is not
needed, the funds will return to the capital fund. He explained that the project
has been appropriately budgeted.
The Committee and staff discussed project timing, the project area, and
construction road closures and the detour route.
Alderman Rummel motioned to recommend approval to City Council the award
a bid for the lowest responsive and responsible bidder for the Old Elm storm
sewer and Forest Hill water main project, to Bolder Contractors in the amount of
$1,548,140. In addition, a 10% contingency is requested for any unforeseen
construction issues that may arise during the project. Alderman Moreno
seconded the motion, which carried unanimously.
IX. REVIEW OF THE UPDATED FY ’20 STREET RESURFACING LIST – BOB ELLS
Superintendent Ells reviewed the below updated list of streets that will be
resurfaced as part of the FY20 program.
He explained that the list had been updated as a result of the City staff decision
to allocate additional funds to the City’s street patching program. He explained
this decision was because of the reports from many residents and staff of large
potholes Citywide. These potholes formed due to the past snow season’s
extreme winter weather
The Committee and City staff discussed the appropriate traffic control signage
for the project.

X. DISCUSSION ON THE POSSIBLE PATH ON THE NORTH SIDE OF ROUTE 60 – BOB ELLS
Superintendent Ells reviewed the discussions and efforts to date concerning the
possible path on the north side of Rte. 60. He explained that the project would
provide safe bicycle and pedestrian access along Illinois Route 60, while serving
as a direct extension of the Middlefork Savanna Trail and a vital link to the
Conway Business Park. He explained that the City submitted a grant application
for the 2017 ITEP call for projects but did not receive an award.

CONTRACT
Award of Proposal to Multisystem Management Company for a Janitorial Services
Contract Included in the FY 2020 – 2022 Operating Budgets for the sum of $617,142.
STAFF CONTACT: Dan Martin, Superintendent of Public Works (810-3561)
PURPOSE AND ACTION REQUESTED: The Public Works committee and City staff are
requesting approval of the recommendation to award a proposal to the lowest and
responsible respondent, Multisystem Management Company, for the janitorial services
contract included in the FY 2020 – 2022 operating budgets.
BACKGROUND/DISCUSSION: In 2016, the City entered into a three year janitorial services
contract with Aplus Building Services. The current three year (FY 2017 – FY 2019) contract
for janitorial services ends June of 2019.
The janitorial services contract encompasses general janitorial building cleaning tasks at
13 buildings throughout The City of Lake Forest covering all of the core facilities. The
agreement includes varying schedules ranging from once a week at the Water Plant to
seven days a week at the Public Safety Building. Building janitorial services are provided
during the evening hours at all buildings. The contract outlines all of the building
cleaning tasks and the frequency rate each task is required to be completed (daily,
weekly, monthly, etc.). The contract also contains twice a year floor cleaning at each
of the 13 buildings.
In May of 2019, City staff initiated a Request for Proposal (RFP) process to obtain
proposals for a new, three year janitorial services contract. On May 4, 2019, an invitation
to submit proposals was published in the Lake County News Sun and on the City’s
website. The invitation explained that firms seeking to submit a proposal would first have
to attend a mandatory pre-proposal meeting/tour of all the 13 City buildings included
in the janitorial service contract. On May 10, 2019, six janitorial firms attended the
mandatory pre-proposal tour. All six firms in attendance at the tour were given the
janitorial services contract RFP package. This package included the contract general
conditions, detailed building information for all of the 13 City buildings, and all the
required RFP submittal forms. Firms were required to submit the required proposal
documentation by May 24, 2019.

BUDGET/FISCAL IMPACT: On May 24, 2019, City staff received the following six proposals
from each of the firms that attended the mandatory pre-proposal meeting/tour:

Has City staff obtained competitive pricing for proposed goods/services? Yes
Per the RFP process, on May 31, 2019, The City staffs interview team selected three firms;
Eco Clean Maintenance, Aplus Building System, and Multisystem Management
Company, to be interviewed. Following the interviews, the interview team
recommended entering into a contract with Multisystem Management Company due
to following reasons:
• Their organization structure and their management style in managing the City
buildings and their building crews. Multisystem identified a sole supervisor that will be
tasked with monitoring each of the 13 City buildings. The supervisor will be ensuring
that each buildings required daily, weekly, monthly, quarterly, and biannual tasks
are completed at the appropriate frequency and to the City’s satisfaction.
• Their approach in meeting with City staff on a regular monthly basis to check in on
building cleaning satisfaction.
• Their overall total cost for the three year janitorial services agreement. As part of the
RFP process firms were required to submit detailed, monthly minimum crew
manpower hours for each of the 13 buildings. These detailed breakouts also
included monthly costs associated with the recorded man power. Staff’s review of
these hours and costs were found to be appropriately allocated demonstrating that
Multisystem had a very good understanding in meeting the City’s contract needs.
• Their floor cleaning pricing of a yearly rate of $29,830 for a twice a year floor
cleaning. Staff estimated that $30,000 was an adequate price to meet the City’s
needs for a twice a year floor cleaning.
• Their references came back very positive. They have municipal contracts with the
Glenbrook High School plus the Villages of Carol Stream, W. Elgin, and Bartlett. The

Carol Stream Police Chief stated “Multisystem is the best cleaning company they
have had in 21 years!”
Eco Clean Maintenance was not recommended as the lowest and responsible
respondent due to the extremely low costs associated with their twice a year floor
cleaning and their detailed, monthly minimum crew manpower hours for each of the 13
buildings. Eco Clean identified their yearly rate of $7,130 for a twice a year floor
cleaning for both carpeted and hard surfaces estimated at 125,000 SF. During the
interview this pricing was called into question. Eco Clean explained that they were
willing to take a loss on floor cleaning if awarded the City’s contract. The interview
team strongly believed that in taking such a significant yearly loss, Eco Clean would be
unsuccessful in meeting the City’s janitorial needs. Eco Clean also failed to demonstrate
any monthly building crew employee adjustments on a monthly basis. Staff expected
qualified firms to allocate additional manpower during the winter months at the core
facilities used by the public. This is largely due to the impact of sidewalk and roadway
salt on the regular building floor tasks. Eco Clean did not record any monthly
adjustments.
Aplus Building System was also not recommended as the lowest and responsible
respondent. This was due to their past contract performance. Staff agreed that Aplus
did not adequately meet the needs of the individual janitorial service building tasks as
well as the twice a year floor cleaning. Specifically, the Public Safety Building and
Recreation Center had multiple issues with missed cleanings.
Therefore, City staff and the Public Works Committee recommend Multisystem
Management Company as the lowest and responsible respondent.

professional engineering design services for the Ringwood Road Ravine Project in an
amount not to exceed $44,500.
BACKGROUND/DISCUSSION: During the spring of 2018, City Water & Sewer Section staff
identified a failure of a stone retaining wall on the north side of Ringwood Road, east of
Sheridan Road. The retaining wall supports a stormwater culvert that conveys
stormwater through the Walden ravine. On April 4, 2019, Bleck Engineering inspected
the headwall and the stormwater culvert at the request of the City. Bleck Engineering’s
preliminary findings confirmed that multiple portions of the stone retaining wall has
collapsed and that another section of the wall has developed a vertical crack. A
portion of the failed wall has also collapsed into the box culvert. The failures have also
resulted in a portion of the adjacent ravine slope sliding. Repairs are needed to re-build
the crushed end of the box culvert, the failed retaining wall, and to correct the ravine
slide that has occurred on the slope. This infrastructure has been confirmed to be all
City owned.

BUDGET/FISCAL IMPACT: On September 4, 2018 City Council approved the City
Manager to enter into a professional agreement with Bleck Engineering Company, Inc.
and Hey and Associates Inc. for professional public ravine engineering services. The
firms were selected via a public Request for Proposal (RFP) process. The City initiated
the process in an effort to improve efficiency and provide consistency on City ravine
projects. As part of the RFP process the selected firms submitted their associated team’s
hourly rates. There are two budgeted ravine projects that City will need professional
design services for in FY20; the Rosemary Road Ravine Project and the Ringwood Road
Ravine Project. It is recommended that each firm is selected to complete one project

At your request we inspected the Walden Ravine culvert and stone walls at Ringwood
Road which is approximately 1,200 feet east of Sheridan Road. There are two stone
walls flanking the northern end of a brick box culvert. It appears that the walls were at
one time connected to the headwall of the box culvert as a means of directing flow to
the pipe, to provide protection of the toe of the ravine, and provide earth retention to
reduce the ravine slope above the top of walls.

The eastern portion of the west wall has collapsed into the box culvert. The failure of
the wall resulted in a ravine to slide to occur above it. This wall provides earth
retention for Ringwood Road.
Several vertical cracks have developed in sections of the remainder of this wall. The
long-term stability if the wall needs to be addressed.
The wall to the north of the box culvert collapsed to the east as shown in the following
photo. This wall failure is less significant than the previous one as the ravine slope to
the east of the wall is less than that of Ringwood Road.

Based on our site visits our understanding of the scope of the design services to repair
the ravine for the long-term includes the following:
a. remove the downed trees, debris and remnants of stone retaining walls
b. repair or replace the retaining wall west of the box culvert
c. re-build the crushed end of the 5’ x 5’ box culvert
d. replace the weir plate / trash rack at the end of the box culvert
e. provide rock riffles and or step pools upstream of the box culvert to slow the
water down in the ravine and reduce the debris buildup at the box culvert
f. re-establish ravine toe protect where the retaining wall collapsed north of the box
culvert
g. repair the ravine slope above the retaining wall west of box culvert, below
Ringwood Road
h. re-establish planting in the ravine for long term stabilization
After the construction of the repairs, it is important to remember that annual
maintenance should be done to prevent debris from building up in front of the culvert.
Bleck Engineering recently completed a topographic survey of the project area. We are
pleased to provide you with a proposal for civil engineering design services for the
Ravine Repair.
SERVICES:
1. STUDY & REPORT
a. Consult with OWNER to define and clarify Owner’s requirements for the
Project and available data.
b. Identify, consult with, and analyze requirements of governmental
authorities having jurisdiction to approve the portions of the Project
designed or specified by ENGINEER, including but not limited to mitigating
measures identified in the environmental assessment.
c. Identify and evaluate alternate solutions available to OWNER and, after
consultation with OWNER, recommend to OWNER those solutions which in
Engineer’s judgment meet Owner’s requirements for the Project.
d. Prepare a report (the “Report’) which will, as appropriate, contain
schematic layouts, sketches and conceptual design criteria with
appropriate exhibits to indicate the agreed-to requirements,
considerations involved, and those alternate solutions available to OWNER
which ENGINEER recommends. This Report will be accompanied by
Engineer’s opinion of Total Project Costs for each solution which is so
recommended for the Project with each component separately itemized,
including the following, which will be separately itemized: opinion of
probable Construction Cost, allowances for contingencies and for the
estimated total costs of design, professional, and related services provided
by ENGINEER and, on the basis of information furnished by OWNER,
allowances for other items and services included within the definition of
Total Project Costs.

e. Engineer’s services under the Study and Report Phase will be considered
complete on the date when the final copies of the revised Report have
been delivered to OWNER.
2. PRELIMINARY ENGINEERING
a. After acceptance by OWNER of the Professional Service Agreement, and
upon written authorization from OWNER, ENGINEER shall:
1) On the basis of the above acceptance, selection, and
authorization, prepare Preliminary Design Phase documents
consisting of final design criteria, preliminary drawings, outline
specifications and written descriptions of the Project.
2) Advise OWNER if additional reports, data, information, or
services are necessary and assist OWNER in obtaining such
reports, data, information, or services.
3) Based on the information contained in the Preliminary
Design Phase documents, submit an engineer’s opinion of probable
Construction Cost.
4) Perform or provide the following additional Preliminary
Design Phase tasks or deliverables:
1. Complete preliminary engineering for site grading and site
improvements
5) Attend meetings with Owner and design team to review
project details, coordination, scope, and schedule.
6) Furnish the Preliminary Design Phase documents to and
review them with OWNER.
7) Submit to OWNER 4 final copies of the Preliminary Design
Phase documents and engineer’s opinion of probable Construction
Cost.
b. Engineer’s services under the Preliminary Design Phase will be considered
complete on the date when final copies of the Preliminary Design Phase
documents have been delivered to OWNER.
3. FINAL ENGINEERING
a. After acceptance by OWNER of the Preliminary Design Phase documents
and engineer’s opinion of probable Construction Cost as determined in the
Preliminary Design Phase, but subject to any OWNER-directed
modifications or changes in the scope, extent, character, or design
requirements of or for the Project, and upon written authorization from
OWNER, ENGINEER shall:
1) On the basis of the above acceptance, direction, and authorization,
prepare final Drawings indicating the scope, extent, and character
of the Work to be performed and furnished by Contractor.
Specifications will be prepared, where appropriate.
2) Provide technical criteria, written descriptions, and design data for
Owner’s use in filing applications for permits from or approvals of
governmental authorities having jurisdiction to review or approve
the final design of the Project and assist OWNER in consultations
with appropriate authorities.
3) Advise OWNER of any adjustments to the opinion of probable
Construction Cost.

performed or furnished under more than one prime contract, or if
Engineer’s services are to be separately sequenced with the work of one
or more prime Contractors (such as in the case of fast-tracking), OWNER
and ENGINEER shall, prior to commencement of the Final Design Phase,
develop a schedule for performance of Engineer’s services during the Final
Design, Bidding or Negotiating, Construction, and Post-Construction
Phases in order to sequence and coordinate properly such services as are
applicable to the work under such separate prime contracts. This
schedule is to be prepared and included in or become an amendment to
Exhibit A whether or not the work under such contracts is to proceed
concurrently.
c. Engineer’s services under the Final Design Phase will be considered
complete on the date when the submittals have been delivered to OWNER.
4. WETLAND DELINEATION, REPORT, AND PERMIT
a. Complete a wetland delineation. We will submit a jurisdictional
determination to the appropriate agencies to identify who has jurisdiction.
b. We will prepare and submit a permit application to the Corps of Engineers
which will include application forms and reports associated with the design
including wetland delineation, alternatives evaluations, mitigation, and
proposed plans. Prepare public notice language to support and help
expedite the agency processes. We have not included any potential
agency permit fees in this proposal.
c. Post application period coordination. We will respond to agency inquiries
for additional information and application clarifications along with
responding to agency and public notice questions.

additional work or other work, preformed at the Clients request will be invoiced in

annum), are added to the unpaid balance. Reimbursable expenses shall be reimbursed
at cost plus 10% administration charge. Such expenses shall include, but are not
necessarily limited to travel, reproduction, shipping/delivery, consultants and
subcontractor fees, equipment and supply costs related to the execution of the project.
Consultant and client agree that the general terms of the agreement for consulting

Exhibit A
General Terms of Agreement for Consulting Services
1. Client hereby agrees to retain Consultant to perform professional land surveying and engineering
services as described herein. This agreement applies only to the specific project identified herein.
2. General Responsibility: Consultant agrees that he shall act as an independent contractor and shall
perform the services provided in this Agreement in accordance with the general standards of care of
Consultant’s profession.
3. Consultant agrees to perform the services set forth herein (services).
4. Client agrees to compensate Consultant for its services according to the schedule of payments as set
forth in Services and Fees.
5. Insurance: For any services provided under this agreement, Consultant shall procure and maintain in
effect insurance coverage in the amount not less than set forth below.
a. Workers’ Compensation and Employers’ Liability: Comply with the laws of the State of Illinois.
b. General Liability: Comprehensive Liability Insurance for personal and bodily injury, including
death and property damage in the amount of $2,000,000 combined single limit each
occurrence and in aggregate.
c. Professional Liability Insurance: Consultant shall provide Professional Liability Insurance in
the amount of $1,000,000.
d. Certificates: Consultant shall provide certificates of insurance upon request evidencing
coverage required above. Each certificate shall provide that coverage afforded shall not be
cancelled nor ordered reduced by the Consultant except with at least thirty (30) days prior
written notice to the Client. Should this occur, Consultant shall procure and furnish for Client
prior to such effective date new certificates conforming to the above coverage requirement.
Consultant shall not have the right to receive any payment under the Agreement until such
certificates are received by Client.
6. Indemnification and Hold Harmless: Client agrees to indemnify and hold Consultant, his employees,
and agents harmless of, from, and against any claims, costs, expenses (including attorney’s fees),
judgments, penalties, liabilities or losses which may be sustained by or secured against Consultant, his
employees’ acts, and agents, arising from the negligent acts, errors, or omissions of the Client, his
agents arising out of or connected with the performance of the Agreement.
7. Termination: Client may terminate the Agreement with Consultant upon written notice if Consultant
breaches his obligations under the Agreement or for any other clause. Client shall compensate
Consultant for performance services through the period of notice.
8. Attorney Fees: Should suit be filed to enforce or for the breach of the term of this Agreement, the
prevailing party shall be entitled to the award of reasonable attorney’s fees.
9. Governing Law: This Agreement shall be governed by the Laws of the State of Illinois.
10. Severability: If any provision of this Agreement is held by a court of competent jurisdiction to be
invalid, void, or unenforceable, the remaining provisions shall remain in full force and effect and are
binding on the Consultant and Client.
11. Bleck Engineering reserves the right to adjust the prices quoted if not accepted within 30 days.

bridge on Rosemary Road is located just east of Thorne Lane. In further reviewing the
area, portions of the wall of the brick culvert that crosses under the Rosemary Road
bridge have collapsed. The brick culvert is an older piece of infrastructure. The material
that is holding the masonry together is most likely weakening. Due to a failure of the
wall, the ravine slope above the adjacent storm water culvert has settled causing the
sinkhole to form. The ravine slope and the storm water culvert are in need of repair in
order to protect the structural integrity of Rosemary Road.

This proposal is for services requested under the overall Professional Ravine Engineering Services
contract between the City of Lake Forest (City) and Hey and Associates, Inc. (Hey). All standard
terms and conditions shall be as defined in that agreement, dated September 14, 2018.
This request for work involves the Rosemary Ravine project. The project will include repairs to
the existing masonry roadway culvert, upstream and downstream gabion walls, sanitary sewer
repairs (if required), roadway drainage improvements to provide 100-year capacity without
overflowing down ravine slopes, headwall repairs, culvert outlet scour protection, and project
access restoration. All work on the ravine bed or side-slopes not associated with infrastructure
improvements is excluded from this proposal. We have already performed initial survey tasks and
coordination and offer the following scope of services to provide engineering design and
permitting services for the proposed project.
TASK 1. WETLAND FIELD INVESTIGATION AND DELINEATION REPORT
We will complete a field investigation of the project areas for wetland and/or Waters of the United
States (WOUS), and perform a routine wetland delineation applying the general procedures detailed
in the 1987 U.S. Army Corps of Engineers’ (USACE) wetland delineation manual and the 2010
Regional Supplement-Midwest Region. We will identify and flag the wetland boundaries for
surveying. We will review the survey for consistency with our field notes.
The report will include an aerial photograph showing the surveyed wetland boundaries, required
USACE dataforms for sample points, observed vegetative species lists, representative color photos,
and other necessary data. We will provide a pdf of the final report to you for your use.
We will complete this task for a lump sum fee of $2,500.

TASK 2. REQUEST FOR JURISDICTIONAL DETERMINATION/BOUNDARY CONCURRENCE
We will prepare and submit correspondence requesting jurisdictional determination/boundary
verification as required by the Watershed Development Ordinance (WDO) and attend one (1) field
meeting with the USACE. The USACE does not require a review fee. Should this task be required,
we will complete it for a lump sum fee of $750.
TASK 3. ADDITIONAL LAND SURVEYING SERVICES
Our team will provide the following land surveying services to supplement the survey work already
completed for the project:

2. Finalize base survey for design, including final stamped version for records and base

preliminary design recommendations. We will evaluate proposed repairs. As part of this task we
will also provide recommendations on multiple repair options, such as pipe lining or replacement
and repairs to gabion walls, full replacement, or alternative options. We will also prepare an
engineer’s estimate of probable cost (EOPC) for the proposed repairs for uses in planning. This
task includes one (1) meeting with City staff to review and discuss project details. We will also
provide a memorandum with the preliminary documents outlining key design information, permit
constraints, and other pertinent project information.
This task also includes preliminary design calculations such as rock stabilization sizing and culvert
capacity calculations. Roadway drainage improvements and sanitary sewer modifications or repairs
are excluded from this task.
We will complete this task for a lump sum fee of $7,500.
TASK 5. CONSTRUCTION DOCUMENTS
Following concurrence on the design approach and extents with the City, we will prepare
construction documents for the project. The construction documents will generally follow Illinois
Department of Transportation standards and City contract formatting. At a minimum, the plans
will contain the following sheets:

The plans will be prepared such that they can be bid to prospective bidders and include all
information anticipated to be necessary for permitting. Construction documents will also include
project specifications and updated EOPCs. We will provide documents for review at
approximately the 75 and 95 percent completion stages. This task includes two (2) meetings with
the City to review the documents at each submittal phase. We will incorporate City comments for
preparation of the 95 percent and final bid documents. We assume “front-end” contract
documents will be provided to us for final review and formatting or prepared by the City. Structural
and geotechnical engineering are included as separate allowances as outlined below, as the extents
of this work are not specifically known at this time. Roadway drainage improvements and sanitary
sewer modifications or repairs are excluded from this task.
We will complete this task for a lump sum fee of $18,500.
TASK 6. STRUCTURAL ENGINEERING
It is likely that some structural engineering tasks will be required as part of the project. At this time
we anticipate this work may include, but is not necessarily limited to:
1. Site inspection of concrete headwalls and steel sheetpile to determine condition and any
recommended repair needs, including a memorandum of findings
2. Structural design support, including stamped calculations and details, of gabion or other
wall design as necessary
3. Assistance with design and specifications for pipe lining/repairs, if determined to be
required/best approach
4. Design of any concrete wall repairs/remediation, including specifications (this item
excludes full wall replacement or major redesign and includes only crack and surface
repairs as determined to be necessary. More major design efforts will be by separate
proposal)
5. Assistance with cost estimates for structural items
6. Attendance at up to two (2) project meetings
We will complete this work on a time and materials basis for a fee not to exceed $15,000.

TASK 7. GEOTECHNICAL ENGINEERING
If it is determined to be necessary during initial design phases, we will coordinate collection of
geotechnical borings to assist with slope and wall designs. We will solicit up to three (3) cost
proposals and relay these to the City for review and approval.
We recommend budgeting approximately $5,000 for this work.
TASK 8. ROADWAY DRAINAGE DESIGN
If requested by the City, we will perform hydrologic and hydraulic calculations to determine flow
rates and existing capacity of the roadway drainages system. We will prepare revised design to
accommodate 100-year capacity without overflow of the roadway to the ravine slopes. This may
include new or improved inlets, additional storm sewer, or improvements to existing storm sewer
system. We will include any improvements, including EOPCs and specifications in the documents
as described above. This task assumes that this work will not be prepared as a separate set of
construction or permit documents.
We will complete this work for a lump sum fee of $5,000.
TASK 9. SANITARY SEWER IMPROVEMENTS
If requested by the City and identified during preliminary tasks, we will prepare provide design
services to make necessary sanitary sewer improvements. This may include manhole improvements
or replacement, pipe replacement or lining, or other identified improvements. Work will be
confined to the portion of sewer located in the downstream culvert embankment and immediately
downstream. We will include any improvements, including EOPCs and specifications in the
documents as described above. This task assumes that this work will not be prepared as a separate
set of construction or permit documents.
We will complete this work on a time and materials basis for a fee not to exceed $5,000.
TASK 10. PERMITTING
It is likely that the proposed stabilization measures will require permitting including:
1. USACE Regional Permitting under Section 404
2. Lake County/ City of Lake Forest Watershed Development Permit
3. LCSMC soil erosion and sediment control review as required by the USACE
Task 10a. Watershed Development Permit
The project will require a Watershed Development Permit. We assume that as a City project, the
LCSMC will be the permit issuing authority for this project. We will confirm this prior to submittal,
but permit requirements will be the same in either case. We will prepare a brief memorandum
report documenting the projects for WDO requirements including soil erosion and sediment

control, flood-prone area construction, in-stream work, and rock sizing. We will also prepare a
Watershed Development Permit application form and submit these documents along with the
plans to the City and LCSMC. Based upon past experience documentation of a Base Flood
Elevation (BFE) or compensatory storage will not be required for the project, as the floodplain is
contained within the ravine and impacts will not occur. This scope excludes these items.
We will complete this task for a lump sum fee of $2,500, including one (1) round of response to
comments.
Task 10b. USACE Regional Permit
At this time, we believe that a Regional Permit will be required for impacts to wetlands or WOUS.
We will prepare and submit a Regional Permit application likely including Regional Permit numbers
5, 7, and 9 to the USACE. The wetland permit applications will include the following:
1. USACE’s Joint Application Form signed by the City as applicant,
2. Project narrative including purpose and need coordinated with the City,
3. Wetland Delineation Report (Task 1),
4. 11 x 17 wetland impact exhibit,
5. 11 x 17 preliminary engineering plans,
6. Correspondence regarding federally listed threatened/endangered species,
7. Correspondence from the Illinois Department of Natural Resources (IDNR) regarding
biological resources,
8. Documentation from the IDNR regarding cultural resources,
9. 3-Year Management and Monitoring Plan for native vegetation areas (if necessary), and
10. We will coordinate the SESC plan submittal to LCSMC.
We will complete this task for a lump sum fee of $4,500, including two (2) rounds of response to
comments.
We assume all permit fees will be paid directly by the City. Any permits not specifically listed are
excluded from this scope of work.
TASK 11. PROJECT MEETINGS, B ID, AND CONSTRUCTION PHASE SERVICES
If requested, we will provide support services during the bid and construction phases of the project.
We understand that the City will largely perform bid and construction management tasks. Services
may include attendance at a pre-bid meeting, answering questions during bidding, assistance with
review of bids, field visits during construction for observation that work is being completed in
conformance with the contract documents, review and approval of contractor submittals, and
participation in a punch-list walk-through. We assume that the Contractor bears all responsibility
for project means and methods, job site safety, and compliance with the construction documents.
This item also includes any project meetings not specifically noted above. We will complete this
work on a time and materials basis. Based on our current anticipation of the nature of the work,
we recommend budgeting approximately $2,500 for these services. We can discuss the role the City

RE: West Fork Culverts Adjacent to Tollway
At the recent request of a fellow neighbor and due to stormwater ponding in backyards, The City of Lake
Forest has completed a review of the storm sewer culverts located along the west side of West Fork Drive and
east of the Tollway (located behind the properties adjacent to the tollway on the west side of West Fork
Drive). City staff would like to share the findings of this review.
There are approximately eight storm sewer culverts located along the west end of the properties on the west
side of West Fork Drive and east of the Tollway. As you may be aware, these culverts were installed in 1988 as
part of an Agreement initiated by the 29 West Work neighbors to construct a berm between the Tollway and
western edge of the West Fork Drive backyard properties for screening purposes.
As part of the Agreement between the neighbors, construction contracting firm, and The City of Lake Forest,
the neighbors agreed to pay an equal share to construct a berm in order to create buffer to the Tollway. The
property owners also agreed to pay equal shares to install the required stormwater culverts along the berm in
order to continue to allow the natural conveyance of stormwater from the berm west towards the Tollway. In
paragraph 16 of the Agreement, the neighbors agreed to be responsible for maintaining the berm and also
hold the City harmless from any and all damages and claims that may arise from such maintenance.
As part of the City’s review, many of the berm culverts were observed as currently being substantially
impeded or blocked. At this time, the City is not requesting or requiring any culvert maintenance to be
completed, however, you and/or your fellow neighbors may be interested in having the necessary work
completed to clear the culverts of any debris or obstructions to better improve the flow of stormwater along
the western edge (the back yard side) of your properties.
Enclosed you will find a map that outlines all the properties who participated in and executed the 1988
Agreement. Also enclosed is a copy of the 1988 Agreement for reference.
According to City records, 19 of the 29 properties have changed ownership since 1988. While the document
was recorded with Lake County, we understand that this may be new information for you. If you have any
questions about the information shared in this letter or if you are interested in meeting to further discuss the

This Contract entered into this 31st day of May, 1988,
by and among the undersigned owners of residences on the west
side of West Fork Drive on the parcels o£ property described in
Exhibit C hereto, (the "Owners"), Lenzini Excavating Company, of
Prairie View, Illinois (the "Contractor") and the City of Lake
Forest, Illinois (the "City")
Witnesseth, that the Owners and the Contractor, for the
consideration hereinafter named, agree as follows:
1. Scope of the Work. The Contractor shall furnish
all of the materials and perEorm all of the work necessary for
the construction of an earthen berm as described on Exhibit A
hereto (the "Project"), such Project to be constructed on the
premises described on Exhibit B hereto and which materials and
work and the plans pertaining thereto to be prepared by Joe
Harding and Troy Miller shall be satisfactory to the Represen-
tatives hereinafter designated and to the City of Lake Forest,
Illinois (the "City").
2. Time of Completion. The work to be performed under
this Contract shall be commenced by Contractor as soon as pos-
sible after the execution of this Contract and shall be completed
as to the items described in paragraphs (a) through (d) of
Exhibit A hereto by a date not later than two and one-half (2-i)
years after this Contract is signed and to be completed as to the
items described in paragraphs (e) and (f) of Exhibit A hereto not
later than three (3) years after the date of this Contract is
signed.
3. Contract Sum. The Owners shall pay the Contractor
for the performance of the Contract the sum o£ One Hundred Ten
Thousand Dollars ($110,000); provided, however, (i) if more or
less than eight culverts for drainage are required, as determined
by the City or the Representatives, and approved in writing by
the Representatives, the Contract sum shall be increased or
decreased, as the case may be, by $2,000 per culvert and (ii) if
the Representatives and the City Engineer shall, after research
and consideration, determine that the topsoil referred in
subparagraph (e) of paragraph 4 hereof shall be deleted from the
Project, the Contract sum shall be reduced by $20,000.
4. Progress Payments. The Owners, acting through
their representatives, as hereinafter designated (the "Repcesen-
tatives") shall make payments on account of the Contract as
follows:
(a) $23,000 upon the completion of the tree removal by
Hendrickson the Care of Trees;

of topsoil;
(f) $26,000 upon the completion of hydro-seeding by ILT
Landscaping and McGinty Bros; and
(g) $3,300 upon the final approval of the berm by the
Representatives as provided for in paragraph 5.
Accompanying each request for payment, the Contractor
shall present to the Representatives, at the address listed
below:
(a) a list showing the quantity and price of materials
incorporated in the work and materials suitably supplied at the
site thereof during the previous month, and other information
reasonably requested by said Representatives; and
(b) a receipted statement trom each subcontractor set-
ting forth the charges made by said subcontractor for work per-
formed under the Contract during the previous month and an
appropriate lien waiver by such subcontractor.
5. Final Payment. The Contractor shall give written
notice to the Representatives, that the work is completed and
shall submit evidence satisfactory to the Representatives, in the
form of receipted bills for all materials furnished, waiver of
liens from subcontractors not previously furnished, and in such
other form as may be required by said Representatives that all
payrolls, material bills and other indebtedness connected with
the work have been paid. The Owners and said Representatives
shall have the right and opportunity to make a final inspection
of said work and materials within ten days after receipt of
notice of completion of the work, and evidence of payment of all
potential lienholders, and upon acceptance thereof by the
Representatives, payment shall be made of the remaining balance
due the Contractor. Such acceptance shall not be unreasonably
withheld and in the event the Representatives refuse to accept,
the Representatives shall within said ten day period notify the
Contractor in writing of such refusal and shall specify the

8 Licenses and Permits The Contractor shall give to
the proper authorities all notices, as required by law, relative
to the work in his charge; obtain all official permits, licenses,
temporary enclosures, obstructions or excavations in public
streets, etc., and also pay such proper and legal fees to public
officers and others as may be necessary to the due and faithful
performance of the work and which may arise incidental to the
fulfilling of these specifications. All materials used, and work
performed, must conform to the laws of the State of Illinois and
ordinances of the City of Lake Forest.
9- Compliance with Laws. The Contractor shall comply
with all building and sanitary laws and all state, county, and
municipal laws, rules, ordinances, and regulations.
10. Deposit Account. The Representatives shall be
authorized to open and close checking or savings accounts, in
banks or similar financial institutions insured by the Federal
Deposit Insurance Corporation or the Federal Savings and Loan
Insurance Corporation, in their names or in the name of a nominee
and to deposit cash in and withdraw cash from such accounts for
the purpose of making payments to the Contractor as provided in
paragraphs 4 or 5 hereof or for the purpose of paying funds to
the_Owners as provided in this paragraph 10. Any"such withdrawal
shall require the signatures of at least two of the Representa-
tives
Interest income, if any, on funds deposited in any
checking or savings account, remaining amounts received from the
Owners which are not necessary to pay for the Project and any
other funds received by the Representatives for the purpose of
constructing the Project will be apportioned equally ~ among the
Owners and returned upon the finaf"completion 'of the Project.
Each of the Owners will be advised, annually of the interest
income, if any, earned on such deposited funds and attributed to
such Owner and each Owner agrees to include such income on the
federal and state income tax returns, if any, of such Owner.
The Owners agree that $4,000 will be deposited with the
Representatives by the Owners of each residential dwelling no
later than May 23, 1988, for the purpose of constructinq the
Project. (1), (2), (3) and (4)

12. Warranties or Guaranties. Neither the Contractor
nor the Representatives shall be deemed to have made a warranty
or guaranty to the Owners that the Project will satisfy, in whole
or in part, the abatement of noise on the premises of the Owners.
The Contractor does warrant that it will for a period
of two years following final payment repair any damage to the
berm resulting from any voids caused by the fill not being placed
compactly in the berm.
13. Designation of Representatives. The following
designated persons: Bob Reaumond, Terry Bellei, Art Nelson,
James Kaufmann, Roderick Miller, Dave Williams and Ted Roth shall
and are hereby authorized to act on behalf of the Owners with
respect to all matters pertaining to the construction of the
Project on behalf of the Owners and the financing thereof.
14. This Agreement shall run with the land and shall
be binding on any future Owners, their successors and assigns.
15. Any notices or documents to be supplied to the
Representatives shall be mailed, postage prepaid, or delivered to

16. The Owners understand that a portion of the
Project will be constructed on land owned by the City. To the

Description of Berm
Specifications and completion periods:
(a) at least ten (10) feet above the level of the road-
way of the Illinois Toll Highway Authority directly
west of the premises within one and one-half (1-i')
years of the date of this Contract;
(b) no more then twenty (20) feet and no less than
eighteen (18) feet above the level of the roadway
of the Illinois Toll Highway Authority directly
west of the premises within two and one-half (2-i-)
years of the date of this Contract;
(c) at least five (5) feet from the fence of the
Illinois Toll Highway Authority, at least eighty-
five (85) feet wide at the base, a slope ratio of
approximately 2 to 1 and not less than 1.9 to 1
minimum with a flat plateau at the top of the berm
at least four (4) feet wide;
(d) composed of not more than thirty (30) per cent non-
clean fill items such as cement, asphalt or aggre-
gate and no toxic waste material, with such'fill
placed compactly and so as to avoid future voids;
(e) the top and sides of the berm to have a minimum
cap-depth of three (3) feet of clean clay fill or
black dirt fill, as determined by the Representa-
tives;
(f) hydrostatic seeding of the berm at the conclusion

2.2 Burr Oak Study Area Existing Conditions
The Burr Oak Study Area is located in northeastern Lake Forest in the Lake Michigan watershed .
The east portion of the Study Area is comprised of multi-family housing with stores and offices,
while the west portion is comprised of single-family residences. It ls roughly bounded by Western
Avenue on the east, Green Bay Road on the west, Alden Lane on the north, and Laurel Avenue on
the south. This area is shown in Figure 2 below and Exhibit 8.1 .

Existing storm sewers ranging from 4 to 24 inches in diameter drain the Study Area to a 36-inch
diameter outlet pipe. The existing system in this Study Area has capacity for the 5-year storm
event. The outlet carries stonnwater across Western Avenue, through the Lake Forest High School
property, and ultimately to the ravine system. The modeling for this Study Area shows significant
flooding on Burr Oak Road south of Thomas Place and in the parking lot behind one of the multi
family housing buildings along Western Avenue (10-tyear flood inundation depths of 0.6-foot and
0.5-foot, respectively). This modeling correlates well with the conditions observed by City staff and
resident reports related to the May 13, 2010 storm event, which shows the closure of Burr Oak
Road and several reports of flooding in the area.

2.3 Burr Oak Study Area Proposed Conditions
The proposed improvements consist of increasing the diameter of storm sewers to accommodate
the peak flow of runoff from the critical duration 10-year storm event. More specifically, the
proposed improvements include:
•Increasing the size of the storm sewers along Burr Oak Road, Thomas Place, and Western
Avenue from 15-21-inch diameter pipes to 36-48-inch diameter pipes; and

•Increasing the diameter of the storm sewers between McKinley Road and Willow Street
from 36-inch diameter pipes to 54-inch diameter pipes.
The proposed improvements, which are shown on Exhibit B.2, would reduce the depth of flooding
on Burr Oak Road south of Thomas Place and in the parking lot behind one of the multi-family
housing buildings along Western Avenue as much as 0.6-foot for the critical duration 10-year storm
event The peak flow rate at the downstream limits of the�roposed improvements wou.ld increase
from 79 cfs to 103 cfs; however, the existing storm sewer system downstream of the improvements
has sufficient capacity for this increase.
The estimated cost of these improvements, including design and construction engineering, is
$740,000 for the improvements west of Western Avenue and $880,000 for the improvements east
of Western Avenue. A detailed calculation of the estimated cost of these improvements is located in
Appendices 1.1 and 1.2. As a Certified Community under the Lake County Watershed Development
Ordinance, the City should be able to authorize the proposed improvements for this Study Area
without permits from any other agency.
